No. MedAid creates the link; you send it yourself, the way you already communicate with patients.

Does the patient have to log in?

No. The link opens in a normal browser, and the patient gives no national ID number.

What if the patient doesn't answer?

Nothing goes wrong. The consultation runs as usual, without a summary.

Do the answers end up in the clinical note?

Only if you attach the summary to the consultation. Then it is read when the note is written, and you approve it as usual.
